Waiver of Claim & Responsibility

Applies to Individual Participants or, for Minors (Under 18), their Parent/Guardian
Note:

Participation in Kids, Incorporated activities requires a signed/acknowledged waiver on file. Registration is void until this document is submitted. This waiver remains valid indefinitely unless revoked in writing and followed by a meeting with Kids, Incorporated officials.

WAIVER OF LIABILITY AND RELEASE:

In consideration of being permitted to participate in Kids, Incorporated activities, I acknowledge and agree to the following:

  1. Understanding of Risks:
    I confirm that I am in good health, physically fit, and capable of participating safely. I accept that Kids, Incorporated’s activities involve inherent risks, including but not limited to serious injury, permanent disability, and death. I assume full responsibility for any risks and related losses.
  2. Release of Liability:
    I release and discharge Kids, Incorporated, its directors, staff, volunteers, sponsors, and affiliated parties (“Releasees”) from any claims, liabilities, or damages arising from participation, including those caused by negligence. I agree to indemnify and hold harmless the Releasees from any costs, including legal fees, related to claims brought by me or on my behalf.
AGE AND IDENTITY VERIFICATION

Kids, Incorporated reserves the right to verify participant age and eligibility through documentation (e.g., birth certificate, report card). For adults, a valid photo ID may be required.

CODE OF CONDUCT

Kids, Incorporated reserves the right to verify participant age and eligibility through documentation (e.g., birth certificate, report card). For adults, a valid photo ID may be required

  • Profanity, abusive language, gestures, or taunting.
  • Aggressive or abusive handling of participants.
  • Acts of violence or unsportsmanlike conduct.
  • Use of drugs, alcohol, tobacco, or vaping at events.
  • Possession of firearms or weapons at events.
PHOTO AND VIDEO RELEASE

By acknowledging this waiver, the parent/guardian or adult participant signature on this form also permits Kids, Incorporated to use still photography and/or video originating from our sporting events or contracted agents for promotional purposes, including but not limited to television and various visual print media.

COMMUNICATION CONSENT

The parent/guardian or adult participant understands that Kids, Incorporated may use the contact information provided to communicate via email, text message, phone call, and other electronic communication. By providing your mobile number, you agree that Kids, Incorporated may send you periodic SMS or MMS messages containing but not limited to important information, updates, deals, and specials. Message and data rates may apply. You will receive up to 4-5 messages per month. You may unsubscribe at any time by texting the word STOP to (806) 230-0663. You may receive a subsequent message confirming your opt-out request. Kids, Incorporated will never charge you for the text messages you receive, depending on your phone plan, you may see some charges from your mobile provider. Please reach out to your wireless provider if you have questions about your text or data plan. By signing, I, the parent/guardian or adult participant, agree to the terms outlined in this waiver and accept all policies and conditions stated herein.

 

Privacy  Policy

No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. All the above categories exclude text messaging originator opt-in data consent: this information will not be shared with any third parties

 

Zero Tolerance Policy

Kids, Inc. will not condone physical violence or unnecessary aggression of any kind. Individuals involved in physical altercations on Kids, Incorporated property are subject to permanent expulsion from our program.

Refund Policy

  • Refund Forms submitted prior to game schedules being posted to our website will be issued, minus a $25 administrative fee. 
  • NO REFUNDS will be issued after game schedules have been posted to our website.
  • If payment was made using a credit card, the refund must be issued back to the same card.
  • If payment was made by cash, check, or money order, Kids Inc. will issue a company check for the refund.
  • If the child was issued a t-shirt or jersey, it must be returned before a refund check or credit card refund will be issued.
It is the responsibility of the parent or guardian to contact the Kids, Incorporated office regarding a refund. Click the button below to complete the Refund Form. Refund Forms are time-stamped when submitted.

Falsification of Information and/or Age – Grade

At any time prior to, or during the season, Kids Incorporated may request proof of eligibility (grade and/or age) for any participant.

Also, a parent/guardian must list the school where the child currently attends, not where they “prefer” to play. Our teams are formed based upon where children attend school. Any deviation from this guideline must be approved by the Director of Programs.
